Decommissioning Procedure
Decommissioning and/or removal of this equipment shall be executed in
accordance to codes, regulations and engineering practices to ensure possible
risk to user, facility, operation and maintenance is reduced to the lowest. Safety
precautions measure shall be considered while performing equipment
decommission.
Following is a general guideline for decommissioning procedure:
1.
Fill in and perform the tasks detailed in Equipment Decommissioning and
Removal Sign-Off check list. The check list shall include following
information:
Date of decommissioning
Reason of decommissioning
Removal of auxiliary equipment e.g. temperature probes
Equipment decontamination (if applicable)
Transfer any confidential information to an alternative storage
medium (if applicable)
Final disposition of the equipment e.g. transferred to, sold to,
recycled, destroyed.
Update disposal information for equipment into Equipment Log
2.
Obtained approval from equipment owner, facility department personnel
and relevant party to proceed.
3.
Attached a defective/scrapped equipment label on the equipment.
4.
Assess the equipment condition.
5.
Disassemble the equipment and salvage serviceable parts where appropriate.
6.
Salvageable parts should be bagged and labeled with reference to the
equipment.
7.
Unusable parts and recyclable parts should be disposed according to waste
policy.
