Ideal appearance:

The plus mark second from
the left, square fourth from
the left, triangle third from the
left, and bar fourth from the
left appear closer than the
other shapes.

Confirm that one of the shapes in each row appears to the patient to be closer.

The stereo parallaxes are 4’, 2’, 1’, 40” from the top.
