Code 2 (1st Acceptance)
Use this code to register:
•
A patient with an NHS GP for the first time.
•
Babies born in Scotland with an NHS GP for the first time.
•
An adopted child whose previous identity can be deleted.
•
A patient who has undergone a sex change, and who has requested that details
of their previous identity are deleted.
Mandatory fields
– GP Code, Surname, Forename, House Name and/or Street,
Address Code, Birth Date and Sex.
Registering a patient with an NHS GP for the first time.
A patient is registered as a first acceptance when his/her GP submits an FP1 and the
patients details do not fit the criteria required to register them as a 1, 3, 4, 5 or 6.
This acceptance code is most commonly used to register children over 12 months old
who have not previously been registered with an NHS GP, and to register patients
who have been treated privately and are registering with an NHS GP for GMS
(General Medical Services) for the first time.
Prints/Notifications Generated
– Z68, FP4 and M/R label prints after a reply has
been received from NHSCR. FP99B electronic notification to NHSCR.
Registering an adopted child so that the previous identity can be deleted.
For adoption procedures, refer to the “Recommended Procedures” section in the
“Introduction to the Registration System” chapter, at the end of Section 1.
Prints/Notifications Generated
– FP4 medical card after confirmation has been
received from NHSCR, FP22, Z68 prints. FP99B electronic notification to NHSCR
should be cancelled.
Registering a patient who has undergone a sex change, and who has requested that
details of his/her previous identity are deleted.
To register a first acceptance, the patient must request that all previous identity details
are deleted.
NHSCR will supply the new NHS number. There is no fixed procedure to deal with
the Medical Record envelope in this situation: actions range from replacing the
envelope (as for an adopted child) to leaving the Medical records as they stand.
