Guidance on Entitlement to the Additional New Start
Weighting
Entitlement to the additional new start weighting (also known as the list turnover
adjustment payment) will apply where patients are transferred from one practice (or
GP) to another when no application to register is made; that is, where a practice
ceases to exist.
The additional new start weighting entitlement will also apply where a GP practice
ceases to exist because a single-handed GP retires or a partnership is dissolved. The
patients may be transferred by the PCT from the old practice to another practice (or
practices). Providing there is no association with any of the GPs at the old practice,
the
New Start
field on the B1 screen should indicate that these are new patients of the
GPs of the new practice (or practices) and the new start weighting will apply.
The new start weighting will not apply in either of the following circumstances.
A practice has split into two smaller practices:
When a practice splits, for example a five partner practice is divided into two new
practices – one with three of the former partners and the other with remaining two, the
partners may decide to take their lists of patients with them to their new respective
practices.
These are not new patients and so it would be inappropriate for the
New Start
field to
indicate otherwise and for the new start weighting to apply.
Equally, the list turnover adjustment should not be applied where patients choose to
see a practitioner of their choice. This might result in the transfer of large numbers of
patients between GPs at the two new practices.
Similarly, patients might wish to transfer from one of the new practices to the other.
In this case, an application to register would be made which would generate an
NHAIS marker for a new patient.
Where a Partner retires from a practice which continues to exist:
A partner might retire from a practice and their list of patients may be distributed to
other GPs within that practice. Again, these are not new patients to the practice. It
would be inappropriate for the
New Start
field to indicate that these are new patients,
or for the additional new start weighting to apply.
