IOPn.MtcScsiP status: DISKn failed a SCSI request, and repeated retries did not succeed. The disk will be marked
faulted.

Error
Description
The SCSI Provider encountered a bad spot while reading one or more sectors from a CE disk.
Action
If the CE disk mirror set is current, then a failover to the correctly functioning disk will occur
automatically. Later, you can use the HP NetServer AA Manager to reenable the faulted disk.
Once the faulted disk is reenabled, a copy to the previously faulted disk will begin, and any
bad sectors should be remapped automatically, making them available for use.
If the CE disk mirror set is not current, then a failover is not possible. The faulted disk must be
restored from backup, as follows.
•
Use Windows NT Disk Administrator in Offline HP NetServer AA I/O Processor Mode to
reformat the disk. Do not select the
Quick Format
option when you format the disk.
Formatting without
Quick Format
will cause any sectors with bad spots to be remapped,
making them available for use.
•
Do not repartition the disk unless the partition table itself has a bad spot.
•
Do not attempt to format the HP NetServer AA partition.
•
Apply backup data after the data partition(s) have been reformatted.
•
Boot the IOP in Operational HP NetServer AA I/O Processor Mode.
Note: The previously faulted disk will still be marked as faulted, and so it must be
explicitly enabled before it can be used.
•
A copy from the IOP with the faulted disk to the remote IOP will begin.
IOPn.MtcScsiP status: DISKn had a previous failure. The device must be manually enabled, using the HP NetServer
AA Manager, before it can be used.

Description
Each time an IOP boots, the state of a mirrored disk during the last time the IOP was in use
will be determined. If the prior state was faulted, the disk will remain in the faulted state until
an administrator manually enables the mirrored disk.
Action
•
Use the HP NetServer AA Manager to enable the disk.
•
If the disk has not been replaced since the reported failure, keep checking the status of the
disk for additional failures.
•
Replace the disk if it continues to fail.
