IOPn.MtcMon status: IOPx was removed at the direction of IOPy.

x – the ID of the IOP being removed
y – the ID of the IOP that directed the removal

Description
The specified IOP was removed at the direction of the remote IOP. This message is generated
when a transition is occurring in the system and an error was detected on the IOP that was
removed. The IOP was removed so the transition could be completed successfully.
