2.4 NIBP Measurement

2.4.2. Non-Invasive Measurement and Monitoring of Blood Pressure
The non-invasive blood pressure measurement channel has three modes of operation, deter-
mined by the type of profile set in the device:
neonatal
,
pediatric
and
adult
. The BP measure-
ment ranges for them are the following:
adult 0 - 300 mmHg
pediatric 10 - 200 mmHg
neonatal 10 - 150 mmHg.
The required profile type is set in accordance with it. 1.3.9.
Before starting blood pressure measurement, check the compliance of the
patient type and the selected cuff with the profile set in the device.
The device performs preliminary BP measurement during the pumping phase This method
avoids over-inflation of the cuff and improves patient comfort. Under normal measurement con-
ditions, the maximum cuff pumping pressure exceeds systolic blood pressure by about 10 ... 20
mmHg. With the patient's motor activity, this value may increase.
After completing the preliminary measurement in the pumping phase, the device measures the
blood pressure when the pressure in the cuff decreases. The final measurement value is deter-
mined taking into account the reliability of each of the results obtained, that increases the accu-
racy of blood pressure measurement.
Measurement during the pumping phase is performed
only for the "Adult" profile
. When using
the "Pediatric" and "Neonatal" profiles, the pumping pressure is determined from the results of
previous measurements.
During the first two to three measurements, the device adapts to the measurement condi-
tions, therefore the best NIBP accuracy is achieved after 2-3 measurements.
In the process of adaptation, a tight fit of the cuff to the body is ensured, the volume of the cuff
is taken into account, as well as the value of the patient's blood pressure and heart rate. Adap-
tation allows to shorten the time for subsequent measurements and adjust to the current volume
of the cuff, which allows the use of shoulder cuffs of different sizes, but the size of the cuff
shall
correspond to the patient's physique
(see it 2.4.1).
.
REMEMBER that the profile type determines the thresholds for the protec-
tion against unacceptable overpressure in the cuff and the maximum meas-
urement duration for different groups of patients - adults, children and new-
borns.
If the device has the option of non-invasive measurement of hemodynamic
parameters (see it. 3.7) and if there is no need to measure these parameters,
to reduce the pumping pressure and blood pressure measurement time, re-
move the NCOV window (see it. 3.7.1).
For the "Neonatal" profile, the maximum pressure in the cuff does not exceed 150 mmHg, the
measurement time limit is 90s. For the "Pediatric" profile, these values are 200 mmHg and
180s. For the "Adult" profile - 300 mmHg and 180 s.
Due to the efficient extrapolation method, the device can measure blood pressure exceeding
the pumping pressure by 15 - 20 mmHg. For example, if you select the "Neonatal" profile, you
can measure systolic blood pressure up to 160 mmHg. If reliable extrapolation is not possible,
the device will display the message "Error: overpressure". In this case it is necessary to change
a profile corresponding to the value of the patient's blood pressure.
When selecting the
“Pediatric” profile for blood pressure measurements of
neonates and infants, make sure that the patient has a really high blood
pressure. The choice of the "Adult" profile in this situation is unacceptable!
