Entering new patient data
Patient data
Patient ID
Enter medical record or ID number unique for this patient.
If
Patient ID
is selected under
Settings>Program>File
Administration
, a minimum of four digits for the Patient ID
is required.
Patient Name
Enter patient’s last name, first name (required).
Birth date
Enter patient's date of birth (xx/xx/xxxx); the software calcu-
lates the patient's age (required).
Note ·
Birth date format dd/mm/yyyy or mm/dd/yyyy is deter-
mined by the main computer setting.
Gender
Select patient’s gender (male or female).
Height
Enter patient’s height in feet and inches or centimeters (for-
mat determined in program settings selection).
Weight
Enter patient’s weight in pounds or kilograms (format deter-
mined by program settings selection).
Address
Enter patient’s address.
Notes
Enter any notes important to the patient’s care.
