ATTENTION: Supervisor
On this date we have entered into a contractual agreement with (the
VP 300 supplier) to be our communications representative
relating to all voice grade telecommunication needs. This agreement is
for an indefinite period.
(The VP 300 supplier) may order connection and disconnection
of telephone-company provided services and equipment. We hereby
acknowledge that (the VP 300 supplier) may obligate us for
installation and other charges that may appear on our telephone bill.
This authorization does not preclude our ability to act on our own behalf
as necessary.
