a previous GP code – use G/‘GP code’
•
a previous partnership code – use P/‘senior GP code’
•
a previous Q code – use H/‘Q code’
•
a previous HA code – use H/‘cipher’ .
The Current Status
Enter a current medical record status code in this field. The valid status codes are:
•
AFH – Awaited from previous HA
•
AGP – Awaited from previous GP
•
APR – Awaiting report from NHSCR
•
CGP – Cancelled record returned to GP
•
EFG – Envelope only to GP
•
EFH – Envelope only to HA
•
FCR – Forwarded to NHSCR
•
FFH – Forwarded to new HA
•
FGP – Forwarded to new GP
•
FMR – First Medical Record
•
NOR – No original available
•
NOT – Not required
•
RFH – Retained at HA.
