where CCYYMMDD is the date that the file was created and HHMMSS is the time
the file was created.
The file will consist of a header record followed by a number of detail records. Each
detail record will contain the following information.

NHS number
•
Failure reason
If the failure reason is ‘Recent Activity’ or ‘New Registration’ the current
demographic details for the patient will also be included. If the failure reason is
‘Recent Activity’ the record will also contain the number of current historic activities
since the date specified in the
Exclude patients who have had their patient record
amended since field. If only one activity is found the patient’s demographic details
from before the change will also be included. This additional information will be
ordered within the record as follows:
