9.20 Controlling an Intra-
Aortic Balloon Pump
(Option)
Option not available in USA and Japan.
In combination with the optional BPI 202
Balloon Pump Interface, PACE 203H
can control the triggering of an intra-
aortic balloon pump (IABP). BPI 202
obtains a signal from PACE 203H indi-
cating a either a ventricular intrinsic de-
polarization or a stimulus and provides a
corresponding signal to the IABP which
resembles a surface QRS complex.
PACE 203H utilizes the intracardiac
electrogram obtained from the ventri-
cular channel. The ventricular signal is
processed in such a way that its charac-
teristics (shape, impulse amplitude, base
width) are similar to a signal obtained
from surface electrodes.
Setup: Connect BPI 202 and PACE
203H via the
CTRL.OUT
terminal of
PACE 203H (see Figure 1 and Figure 2).
Attach the clips of the 3 leads provided
by the IABP which usually are attached
to 3 surface ECG electrodes to the 3
pins provided at the front of the BPI 202.
PACE 203H accomplishes the synchro-
nization of the IABP with the heart activ-
ity in both intrinsic and paced ventricular
events by application of a differential
timing:
Intrinsic Event
: Upon sensing of an
R wave, PACE 203H provides a control
signal, whose processed ‘R wave’ is de-
layed by approximately 5 ms, assuming
that the amplitude of the R wave meas-
ured is twice the value of the sensitivity
set.
Paced Event:
Upon delivery of a
ventricular stimulus, PACE 203H pro-
vides a control signal, whose processed
‘R wave’ is delayed by approximately
35 ms, accounting for the latency period,
that is, the time between actual delivery
of the stimulus and the occurrence of
ventricular depolarization.

Warning:
Because the ventricular
intracardiac electrogram is processed to
control the IABP, the output at the ter-
minal CTRL OUT is only enabled if a
pacing mode with sensing and / or
stimulation in the ventricle is chosen.
The output at the terminal is not avail-
able at atrial operating modes e.g. AAI,
A00, AAT.
Enable the balloon pump interface
communication by selecting the radio
button assigned to BPI (manufacturer’s
default) in the Options Menu 2 (Table
57). Pressing the PMC/BPI softkey will
toggle between selection of the radio
button assigned to PMC (for
Pacemaker
Clinic
™, a program view of the
AESCULON
®
) and the radio button
assigned to BPI (for
Balloon Pump
Interface
). Balloon pump interface
communication is default.
