4.3.8 Remove the template(s). Add the wooden stakes and level them.
4.3.9 Place the wire mesh (6 x 6, 4 gauge). Be sure the wire mesh is centered vertically.
Note: No. 3 (

-inch or 9 mm) or larger reinforcing rod, placed on 15-inch (38.1 cm)
centers, may be used in place of wire mesh.
4.3.10 Place the MESA
®
6 NxT / Calix ODC-120 template in the form and over the conduits. If used,
place the JuiceBox template into the form and over the 1

4
-inch conduit.
4.3.11 Fasten the template(s) to wooden stakes as shown in Fig. 10.
CAUTION
Make certain the MESA
®
6 NxT / Calix ODC-120 cabinet template is flat and level to prevent
misalignment of cabinet doors.
4.3.12 Square and level the template(s) on the stakes so the tops of the mounting plates are flush with,
or no more than

-in. (6 mm) above, the top of the pad (Fig. 11). Square the template(s) so
the diagonal measurement between the anchor bolts is equal.
Note: The cabinet must clear the finished concrete surface so that the base panel below
the cabinet end chamber can be removed.
4.3.13 The conduit should extend approximately two inches (5.08 cm) above the template (Fig. 11).
Cover the conduits at the top to keep concrete from entering them during the pour.
Note: Make sure the conduit ends extend vertically through the template (perpendicular
to the template surface).
4.3.14 Before pouring concrete, be sure that all eight

" x 13 anchor bolts in the MESA

are threaded all the way down in the anchor nuts.
