Obtaining the Data Collection Forms
To access these forms, go to any Install Tool online help page, click on TOC (a
link appears on the top and bottom of each page), then select "Install Tool Basics".
This page provides a link (“Collecting Configuration Data”) to a series of pages
that you can print out and use for collecting your data.
Sample Data Collection Form
This is an example of a data collection form:
System Configuration Sheet
This configuration form is provided as an aid in specifying the values to be set in the Passport
4400 InstallTool System Configuration Panels. Print a copy of this form and write in the
correct values for the Passport 4400 to be installed. Then, while using the InstallTool, use the
values specified when prompted for values by the tool.

Passport 4400 IP Address
/ Passport 6400 DNA
Address
This field specifies the unique
name for the the node (maximum 50
characters).
This field specifies the unique
Device Network Address for this
node. Minimum of 4 digits,
maximum of 15.
This field sspecifies the
operational mode of the node.
Choices are "Branch-Node".
"Regional-Node", or "Central-
Node".
If there are no Passport 6400's in
the network, choose "Server on
Passport 4400", otherwise choose
"Server on Passport 6400".
This field specifies the Voice
Phone Directory Server: either a
Passport 4400 IP address or a
Passport 6400 DNA address. One
node in the network will act as
the address server for voice calls.
