NOTE: Effect site and Plasma concentration will match during the course of infusion.
Nevertheless there are fundamental differences between the plasma target mode and the
effect site target mode. The pump is programmed in a way to reach the target concentration
as fast as possible, without exceeding it. Since the transport of the drug from plasma to effect
site takes longer than from the pump into the plasma, the initial bolus is a lot smaller when
working in plasma concentration target mode. This causes the patient to fall asleep later. To
shorten this time, very often the plasma concentration target is increased at the beginning of
the infusion, causing the effect site concentration to increase as well. This way the patient
will fall asleep almost as fast as when working in effect site concentration target mode.
Warning: When working with drugs as Propofol (Disoprivan®, Recofol®) the initial
bolus when starting the infusion or when target concentrations are increased can
cause a big drop of the patient’s blood pressure. It is in the responsibility of the
nurses to be prepared for taking counter measures.
The last step is to enter the necessary target concentration (in

g/ml).

NOTE:
When working with plasma target concentration „cpt“ instead of “cet” “cpt” must be
entered.
After the infusion set is loaded and all TCI parameters are entered correctly the patient can
be connected and the infusion may be started by pressing „Start/Stop“.
In case there are between preparation of the pump and the start of the infusion more than 4
minutes, the pump will give an inactivity alarm. To disable this alarm or to adjust the inactivity
time, contact your
arcomed dealer.
3.3.8.2 Functions while the Pump is Running in TCI Mode
After starting the pump an initial bolus is delivered. Depending on the settings this is smaller
or bigger. After this bolus, normally the pump will stop for a while to allow a first equalisation.
Then the pump will start at a much lower rate, which is adjusted constantly. If the target
