Options while Pump is Running in PCA Mode
To deliver a patient bolus, the patient has to press the patient button. After that patient bolus,
the lockout time starts. Between patient boluses a doctor can always give a physician bolus
by going to the bolus menu (see chapter 3.5).
Pressing ESC while the pump is running, information to denied and delivered patient boluses
can be displayed in the LCD. This information is also available in the help/status windows.
The pump can be stopped any time by pressing the “Start/Stop” key. When the pump shall
be turned off, the password (1001) has to be entered.

NOTE:
The number of configuration steps in the mode PCA / EPCA is configurable. Contact
your
arcomed
distributor for an adjustment according to your requirements.
3.3.8
TCI Modus - general information
The TCI mode is designed to deliver analgesic drugs during anaesthesia. Running in TCI
mode the pump permanently recalculates the necessary infusion rate to meet the target
concentration (plasma or effect site) as precisely as possible.
Warning: Using the pump in TCI mode, profound knowledge about the different
infusion parameters and how they depend on each other are necessary. The user
must be aware of TCI specific wording such as effect site concentration, plasma site
concentration and their interactions. In case you do not have profound understanding
of TCI you must not use the pump in this mode.
The drugs that are usually used for TCI applications are presafed in the pump: Propofol,
Fentanyl and Remifentanil together with different concentrations. The drugs can be
administered using 3 different algorithms: Schnider, Marsh and Minto. More models are
available, however it’s in the clinicians responsibility to assure the model used is validated for
a particular application and region. Refer to publications and clinical studies available for the
various models.
During application Effect site and plasma site concentration is displayed. The pump also
calculates when the patient should wake up again.
Warning: Not every patient is suitable for a TCI treatment. It is in the responsibility of
the nurses and the anaesthetist to decide if a patient is contraindicated for TCI
treatment.
Warning: Before starting the infusion, make yourself aware of possible side effects of
the used drugs and be prepared to take necessary measures to prevent harming the
patient.
