Preparation
Information on using bacterial filters, HME systems and hose systems
Any additional components in the breathing system
and any components that do not belong to the
standard hose system can substantially increase
inspiratory and expiratory breathing resistance
beyond the standard requirements (e.g. bacterial
filter, HME).
Breathing resistance in the patient connection port
cannot be directly monitored by Carina.
Therefore:

The condition of the patient and the ventilator’s
measured values for air volume must be
checked more frequently.
z
Follow the Instructions for Use for the used filter
and components.

Observe when using heat and moisture
exchangers (HMEs)
If a heat and moisture exchanger (HME) is con-
nected to the patient port, this can increase breath-
ing resistance substantially. An increase in
breathing resistance will in turn lead to greater
effort in spontaneous breathing and/or greater trig-
ger effort during assisted ventilation. Under unfa-
vourable conditions, an increase in breathing
resistance can lead to an unwanted intrinsic PEEP.
This breathing resistance in the patient connection
cannot be directly monitored by the ventilator.
Therefore:
z
The condition of the patient and the ventilator's
measured values for air volume must be
checked more frequently.
z
Follow the Instructions for Use of the heat and
moisture exchanger (HME).
z
Position the filter near to the patient.
Hose systems
WARNING
Always use an inspiratory bacterial filter or
HME/F to reduce possible airborne or liquid-
borne cross contamination of the device.
Otherwise this can result in cross contamina-
tion of the patient.
WARNING
Do not use the heat and moisture exchanger
(HME) together with a medication nebuliser or
humidifier! This can lead to a greater breath-
ing resistance.
WARNING
Using a HME at high relative ambient humidity
can increase breathing resistance substan-
tially. Carina might not indicate a disconnect
situation under these circumstances! Please
check the need of using a HME under these
conditions or connect an appropriate external
monitor to hose system.
NOTE
When using a hose system with expiratory valve
coupled with heat and moisture exchangers and
filters, vibrations may occur during ventilation.
WARNING
The inspiratory breathing gas is warmed by
the built-in blower. To ensure appropriate
cooling of the breathing gas, the total length
of the inspiratory hoses must be not less than
1.2 m. If hoses are shorter, the temperature of
the breathing gas may be too high and thus
put the patient at risk.
