If you are not using DHCP to get the initiator name, clear

and enter a iSCSI Qualified Name (IQN) in the Initiator name field.

DHCP vendor Id
specifies the device vendor ID to be advertised to the DHCP server. This
setting is always enabled and not affected by any of the other DHCP options. See
for more details on this and other DHCP options.

IPv4 Address
An IPv4 address to assign to the adapter. Ensure this address is
unique.
Subnet mask
Subnet mask. For example 255.255.255.0
Default Gateway
IPv4 address of your network router.
Primary DNS
IPv4 address of your Primary DNS server.
