Report on the first start-up of the boiler

(Copy of the owner of the central heating boiler)

In order to verify the purchase and recognise the validity of the guarantee, a report must be sent within 30 days
from the date of first use. This can be done by:

1.

E-mail - in which a scan or photo of the report will be included.

2.

Letter - in which a copy of the report will be sent to the company METAL-FACH Jacek Kucharewicz, the
address of the company can be found at the end of the Technical and Operational Document.

3.
