4.5.1 Rapid Cool

Once evaluation is complete and
the lungs have been accepted for
transplantation, the cooling pro-
cess begins:
Attach funnel tube to cool
-
down basin at the point shown to
the left of the number in picture.
Fill with approximately 200ml of
distilled water until coil is primed.
Disconnect the heater/cooler
blue hose from the left connector
on the panel and attach it to the
same connector where the funnel
was attached.
Connect the hose from the
cooling basin to the
left
heater/
cooler hose panel.
NOTE: to help easily discern hos-
es in this picture, the heater/
cooler hoses are thick blue and
the cooling basin hose is thin
blue. In the clinical system, hoses
will be the same diameter.
Set temperature of heater/
cooler to 15
o
C.
Fill cooling basin with ice and
add water until ice is covered.
Run until STEEN Solution
TM
tem-
perature is 15
o
C (approx. 7
minutes).
NOTE: If lung is not cooling fast
enough,
1)
add more ice.
2)
Check HCU set temp (15
o
C).
3)
Make sure HCU pump is
running.
4)
Consider cold Perfadex®.
The lungs should be clamped
moderately inflated with 50% O
2
similar to the standard donor
procurement protocol.
Disconnect lungs from system
and store in sterile bags with
Perfadex
®
.
Place sterile bags in insulated
cooler containing ice.
Finishing EVLP

NOTE: An alternative method for rapid cooling that does not require the cooling
basin is as follows:
1)
Set the heater/cooler to 15
o
C.
2)
When temperature falls to 32
o
C, clamp the ET tube when the lungs are 50%
inflated.
3)
Disconnect lungs from the perfusion circuit and flush with two liters of cold
Perfadex. Place in sterile bags with Perfadex. This method is very similar to a
standard donor procurement protocol.
