4.0 STEP THREE – REVIEWING THE TEST REPORT
The Test Report
Once the testing sequence is completed, the instrument will print a final Test Report. The report is
characterized by the statement
“Breath Test Report” on the header of the document and contains all
of the information from the testing sequence. Generally, the Test Report will contain the following
information:
1. The testing location.
2. The date of the test.
3. The serial number of the instrument used.
4. The subject’s name, date of birth, license
number, and state of issue.
5. The test method steps, the intermediary
results of each step, the status of each step, and
the time each step was initiated.
6. The final result of the test.
7. Any additional information for the operator.
8. Any “Suggested Corrective Action” information
would appear in this location.
9. The operator’s name, permit number, and
the OAR reference to the approved testing
method.
10. The statement affirming that the operator has
been trained to operate the Intoxilyzer 8000 using
the approved testing method.
11. Any additional comments or observations made
by the operator.
Test Reports are divided into two categories: Complete Test Reports and Incomplete Test Reports.
Complete Test Reports
Complete Test Reports will contain one of three results in the Test Result area of the report:
