Inhoud van de verpakking

Het neurostimulatorpakket bevat de volgende artikelen:

• Neurostimulator

• Torsiesleutel

• Systeemregistratieformulier

• Patiënt identificatiekaart

• Implantatiehandleiding voor de neurostimulator (dit document)

De inhoud van de binnenverpakking is STERIEL. De inhoud van het neurostimulator pakket  mag uitsluitend eenmalig gebruikt worden.

Systeem registratieformulier en patiënt identificatiekaart

Het systeem registratieformulier registreert  het apparaat en vormt wordt opgeslagen in het implantaat gegevenssysteem van Axonics.
De patiënt identificatiekaart wordt verpakt met. De patiënt wordt geadviseerd dit altijd bij zicht te dragen.
