V. DO NOT FORGET
IV. 1. Safety and protection
IV.1.1. Additional air inlet
A valve allows the patient:
•
to breath ambiant air via
OSIRIS 2
if the unit is stopped
(voluntarily or accidentally),
• and, to breath ambiant air if the ventilation flowrate is
insufficient.
IV.1.2. Pmaxi safety
Pmaxi safety allows pressure limitation and therefore avoids
any barotrauma (inspiratory phase stopped, proceeding
to expiratory phase when the set threshold is exceeded).
This can be set to between 10 and 80 cm H
2
O.
IV.1.3. Pmax safety valve
This limits the pressure in the patient circuit to 90 mbar
(cmH
2
O) in the event of a Pmaxi safety failure.
IV.1.4. Gas filtering
This is assured by:
•
a foam pad on the ambiant air intake.
• a nylon filter on the O
2
supply.
A bacteriological filter (type Pall BB50) can be fitted
either on the patient circuit output connection or on the
"patient" outlet of the expiratory valve.
IV. 2. Alarms
Pmini alarm: Intermittent audible and visual alarms will be activated if pressure in the patient circuit does not reach the
set Pmini value (from 5 to 10 mbar (cmH
2
O)) during 2 respiratory cycles.
The audible alarm may be inhibited for 2 minutes by pressing on the
button.
Failure/Message
Audible
signal
Probable cause
Proposed corrective action
F max ALARM
Intermittent
The respiratory rate measured by
OSIRIS is higher than the Fmax
threshold set by the user
Increase the value of the alarm threshold or
resolve the problem of tachypnea
Autotriggering of the inspiratory
trigger
Decrease the sensitivity of the inspiratory trigger
PI < Pmin ALARM
Intermittent
The pressure in the patient
circuit does not exceed the
threshold (PEEP + Pmin) set by
the user for inspiration, for at
least 2 respiratory cycles
Decrease the Pmin threshold, or increase the
set VT, or the set PI.
Patient circuit leak
Ensure the connections are leaktight
Leak at expiratory valve level
Fit the diaphragm correctly or replace it
Probe’s cuff not leaktight
Inflate the probe’s cuff and check that it is
leaktight
No O
2
supply, O
2
cylinder
empty
Connect the O
2
or replace the O
2
cylinder
Expiratory valve control tube
clamped
Check the expiratory valve control tube
Pmax ALARM
BEEP
The pressure in the patient
circuit has reached the set
Pmax value, for one or two
respiratory cycles
Pmax ALARME
Intermittent
The pressure in the patient
circuit has reached the set
Pmax threshold value, for at
least three respiratory cycles
Increase the alarm threshold value or decrease
the set VT
or the set PI
Machine anomaly.
Ventilation shutdown.
Patient exposed to ambient
air
Continuous
Electronic failure

P. max anomaly
Intermittent
Electronic failure
Ventilation can continue but without pressure
monitoring
Monitor the patient closely.
