25 Service form
This form enables the quick processing of repair orders. You should describe
the defect(s) as precisely as possible so that it is easier and faster for the
Service Center to identify the cause and complete the repair.
If a repair / inspection / update is necessary or requested, then a completed
service form must be included with the fusion machine. If you send us a copy
of the service form by fax in advance, we would be able to plan the necessary
repair work and process the repair order faster.
If a repair / inspection / update is necessary, send the fusion machine to your
nearest George Fischer Service or Maintenance Center.
