Select the 

Connect to the network at my workplace

 option and press the Next button.

Select the 

Dial-up connection

 option and press the Next button.

Optional: If you have multiple modems installed on the computer, a prompt will appear asking
which Modem and COM port should be used.  If only one port is set up as a Modem, this screen
will not appear.  Select the modem (

Standard 33600 bps

) and press the Next button.

Type in the name of the connection (e.g. System 5000 Connection) and press the Next button.

Type in 

5000

 as the phone number and press the Next button.
