You can attach the guide head when using the forward-view optical adapter.
There are two types of guide head: One for optical adapter 120D and the other for optical adapter
220D. The guide head for optical adapter 120D has a positioning hole

Be sure to use the guide head for optical adapter 120D.
1.
Attach the optical adapter
(B)
to the distal end.

2.
Rotate the cap
(D)
to remove it from the base
(C)
of the guide head.
3.
Insert the base
(C)
into the distal end from the smaller diameter. Align the groove
(G)
of the guide head with the groove
(H)
of the insertion tube so that one of the D cuts (4
positions)
(F)
of the distal end is visible from the hole
(E)
of the guide head, and hold
the base and insertion tube.
It does not matter which of the four D cuts
(F)
is used.
