PM Form Printing
To complete the PM Form perform the following:
1.
Save the PM Form to your hard disk using the serial number of the 9800 system if you have not already done so.
2.
Print the Form and sign it.
3.
Obtain the customer’s signature.
4.
Make two photocopies of the PM Form.
5.
Mail the copy with the original signature to GE OEC Medical Systems, Inc.
6.
Leave a copy with the customer.
7.
Keep one copy for your records.
