REQUIRED DOCUMENTATION
As part of a ZOLL Certified installation, forms 1-4 are required to be completed and submitted to ZOLL Support for
inclusion in the account records.
Form 1, Vehicle Damage Inspection Form
- Document any areas where damage
or an unusual condition exists, and should be reflected on form 2, the vehicle inspection
form. Bringing these items to the attention of the responsible customer representative
before work begins helps protect the installer against potential claims after the install, and may be unknown to the
customer’s management. Any exception to not completing this form in the presence of the customer must be
agreed upon prior to beginning work. Such an agreement may be approved during the pre-deployment scope call,
upon arrival at the customer’s installation location, or similar, and any documentation of the agreement should be
in writing.
Form 2, Vehicle Inspection Form
- To be used in conjunction with Form 1, the
vehicle inspection diagram, both before starting work, and again upon completion. This
will help prevent installers spending time attempting to troubleshoot and repair
conditions that were pre-existing, and identify vehicle systems that may have been impacted during the install and
will need to be corrected before turnover to the customer.
Form 3, Build Sheet
- is to be used to document wiring locations and specific wires
used during the installation. Submission of this form helps ZOLL build a database of
known good connections for future use, and for assisting installers in the field in need of
connection information.
Form 4, Post Installation Verification Worksheet
- is to be used before, during,
and upon completion of the installation to help prevent the installers from missing any of
the critical steps along the way and to show the customer that the vehicle is complete
and operating normally. Anything not able to be completed such as road testing the vehicle should be annotated
in the notes section describing the reason.
Form 5, Serial Number Assignment Form
- is used to document installed serial
numbers for use in ZOLL Online setup and for archiving after being sent to Road Safety
support upon completion.
These five forms will should be completed for each vehicle and shall be returned either by mail, fax, or scanned
and emailed to ZOLL Support using one of the following:
