1. Call Pacific Scientific at (978) 988-9800 from 8:00 a.m. to

5:00 p.m. Eastern Standard Time to get a Returned Materials
Authorization Number (RMA#).

Note: Do not attempt to return the drive, motor or any other
equipment without a valid RMA#. Returns received without a
valid RMA# are not accepted and are returned to the sender.

2. Pack the drive in its original shipping carton. Pacific

Scientific is not responsible or liable for damage resulting
from improper packaging or shipment.

3. Ship the 5345/5445 to:
