It is important that you carry your

list of your medications with you
at all times.
In a medical emergency, the Implanted Medical
Device Identification Card
contains information of great
importance to an attending physician and will assist in expediting
any emergency medical care you may require

In addition, it is important to notify all your health care providers

device implanted
