Arm not counterbalanced correctly

Arm inadvertently slides down

Set Screws (4) in Slide are loose. 

Reposition Arm, tighten set screws in 
slide, and secure adjustable stop (section 
5.2 and 5.4). 

Adjustable Stop is loose or 
missing. 

Install or secure adjustable stop (section 
5.2). 

VHM-PL (locking) handle actuation 
does not operate correctly (arm will
