Pre-Existing Injury and Illness Notification
Dear Candidate,
It is necessary for The White Ant Co Pty Ltd as your employer to provide a safe workplace
and to prevent injuries or illnesses from re-occurring or being exacerbated by work
conditions. Therefore, it is necessary to request information in regards to pre-existing
injuries and/or illnesses that may impact on your ability to perform tasks as a White Ant Co
Installer. We request that you complete and sign the form on the rear of this letter and
return it to The White Ant Co with your other documents.
The White Ant Co also has a requirement to meet its WorkSafe commitments and as such,
you will not be able to accept a position with us without completing this form.
