Disinfection Declaration
The following Disinfection Declaration MUST be completed by the operating
authority and attached on top of the package in which the instrument is returned,
before sending it to the distributor or service center.

I declare that the outer surfaces and the plate transport of the instrument in
this package have been disinfected to remove or inactivate any biological
material, patient samples, positive control samples or hazardous material,
which could be dangerous to any personnel, or that it has never been
exposed to any hazardous biological material.
