Correct the program.

The output circuit in the SmartAXIS is
damaged.
Replace the SmartAXIS.

SmartAXIS Pro: Check the output with WindLDR
or the device monitor.

Click the Start button in WindLDR on a
computer connected to the SmartAXIS.
