Completing this Form
For Direct Extension Transfer:
1. In the “Description” column, write “Direct Extension Transfer.”
2. In the “Range of Extensions for Direct Extension Transfer” column, leave
the factory default.
3. Leave blank the “Ext. # or Group # for Selector Code Transfer” column.
For Selector Code Transfer:
1. In the “Description” column, write the name of the person or group to
receive the transfer.
2. In the “Range of Extensions for Direct Extension Transfer” column, cross
out the factory default if you want the caller to dial the corresponding
Selector Code to transfer to the extension or group identified in the “Ext. #
or Group # for Selector Code Transfer” column.
3. In the “Ext. # or Group # for Selector Code Transfer” column, write the
extension or group number to receive the transfer.
