like you to complete. Please be sure to write your
name, department and date on the quiz.
When you are finished with the quiz, please turn it
over and wait until everyone is done with it.
CORRECT QUIZ:
Since everyone is done with his or her quiz, please
switch quizzes with a person next to you or behind
you. (If only one employee is being trained, correct
the answers as you review them with the employee.)

FOLLOW-UP:
Check the quiz and ensure each employee passed
with an 80% score or better. If not, those employees
need to repeat the training. Answering 14 or more
questions correctly is a score of 80% or better.
Be sure you record on the bottom of the employee’s
quiz whether they displayed the ability to operate the
WheelChair Mover based on their hands-on training.
Lastly, place quiz in employee’s personnel file.
