Fields in the Patient Registration Form
The Patient Registration form includes the following sections: PATIENT ,
INSTITUTION , EXAM , and HISTORY . Use the system presets to select the
required date format and to create entries for the INSTITUTION section.
The selected date format is displayed to the left of the Date of Birth field.
Patient Data
In this field: Enter the...
Last Name Patient's last name.
If a last name is not entered, then the system generates the
name "Unknown."
Note: The patient name ( Last Name , First Name , and Middle Name
combined) is limited to 60 characters.
First Name Patient's first name.
Middle Name Patient's middle name.
Patient ID Identification code for the patient.
If an identification (ID) is not entered, then the system generates a unique
identifier starting with the system date and time.
This ID code displays on the image screen. If there is not enough space for
the entire code to display on the image screen, a portion of the code is
displayed followed by three dots (…).
Date of Birth Patient's date of birth using the date format selected in the system
presets.
When you enter or change this value, the system calculates and displays
values for
Age .
If a date of birth is not entered before the examination is completed, then
the system generates the date of birth "11/18/1858," but does not display
this date on the form.
Age
The system automatically calculates and displays the patient's age based
on the
Date of Birth , if entered.
Sex Patient's sex.
If

is not selected, then the system selects

.
Height Patient's height using the measurement system selected in the system
presets:
Metric or U.S.
Weight
Patient's weight using the measurement system selected in the system
