Danger to Personnel and Equipment: Rotating the plug counterclockwise can
strip the threads and allow hot gasses or fluids to escape, which will result in
serious injury. Do not rotate the plug counterclockwise.
14. Add proper lubrication to the external threads on the access fitting and the sealing
surface, then install the cover (and plug pipe, if applicable).
WARNING
Risk to Personnel and Equipment: If the o-ring for the pressure retaining cover is
not properly seated against the sealing surface on the access fitting, gas or fluid
might escape and cause serious injury. Ensure that the o-ring for the pressure
retaining cover is properly seated against the sealing surface on the access fitting.
4.3 Troubleshooting guidelines

Procedure
1. If the service valve leaks, check if it is properly closed. Also check that the by-pass
valves are closed. If leaks in the valve cannot be stopped, either the plug or the
probe must be reinstalled, and the service valve must be inspected for further
repair, if applicable.
2. Inspect the o-ring in the bottom of the service valve and verify that it is properly in
place. If necessary, replace the o-ring before proceeding.
4.3.1 Pipe plug is difficult to loosen
Pipe plug is difficult to loosen
The pipe plug should be easy to loosen. If they are difficult to loosen (high friction), there
might be pressure in the plug body. This condition indicates that either primary packing or
probe packing (if applicable) are not sealed properly.
Recommended actions
1. If there is pressure behind the pipe plug even after turning it a ½ turn, don’t
attempt to remove the pipe plug.
NOTICE
Be careful when entering the plug onto the access fitting so that the threads are
not damaged.
2. Re-tighten the pipe plug and abandon the location until the system is shut
down.
