before returning the endoscope for
repair, contact Olympus Customer Service:

Customer Service will send through the relevant paper work and job number.
Please include the following with the endoscope:
Description of the malfunction or damage
Name and telephone number of the individual at your location who is most familiar with
the problem
Repair purchase order
When returning the endoscope for repair, follow the instructions given in ”Transporting
outside the hospital” which can be found in the Olympus’ manual for the endoscope being
returned- reference- Olympus operational manual.
For 180 series: For example, GIF-H180, CF-H180,
twin channel gastroscope GIF-2TH180,
duodenoscope TJF-180V and single balloon
enteroscope SIF-180
