Feeding too slow.
Not lubricated.

Retract bit frequently to clear chips
Sharpen or replace bit.
Feed fast enough; allow drill to cut.

Excessive drill runout or wobble

Worn spindle bearings.
Replace bit.
Install bit properly.
Install chuck properly.
