3. Description of the problem.

If  the  instrument  is  to  be  returned  to  New  Focus,  you  will  be  given  a  Return
Material  Authorization  (RMA)  Number,  which  you  should  reference  in  your
shipping  documents.  Please  fill  out  a  copy  of  the  service  form,  located  on  the
following  page,  and  have  the  information  ready  when  contacting  Newport
