Photocopy this form and use it to record the preventive maintenance performed on the treadmill. Each copy of the 
form can be used for six months (26 weeks). When maintenance is performed, write the date in the appropriate 
spaces. 

IMPORTANT: Perform all maintenance procedures as described on pages 25 to 28. If all proce-

dures are not performed as described, components may wear excessively, the treadmill may become 
damaged, a risk of injury or a risk of fire may result, and the warranty will be voided.
