Unit serial number
•
Description of the problem
•
Department where the equipment is used and the name of the person to contact
•
Purchase order to allow tracking of loan equipment
•
Purchase order for a unit with an expired warranty
Returning a Unit for Service
Before sending a unit to the ZOLL Technical Service Department for repair, obtain a service
request (SR) number from the service representative.
Remove all batteries from the unit. Pack the unit in its original container or equivalent
packaging. Be sure the assigned service request number appears on the package.
