Too much food in the cooker
Take some of the food out
Heat is too high
Use a smaller burner and turn down
the heat
Steam escapes around the lid
Safety and pressure valve not working
Take the pressure cooker off the heat
and let it cool. Clean the regulator

Close the lid properly.
