Type in a new patient ID to create a new patient
profile and tap the SUBMIT button to proceed to
the patient profile page.
Do not use a patients’ Personally Identifiable
Information (PII) as part of patient ID.
Enter height and weight to nearest cm or ft/inch
and kg or lbs.
