Photocopy this form and use it to record the preventive maintenance performed on the incline trainer. Each copy
of the form can be used for six months (26 weeks). When maintenance is performed, write the date in the appro-
priate spaces.
IMPORTANT: Perform all maintenance procedures as described on pages 28 to 31. If all
procedures are not performed as described, components may wear excessively, the incline trainer may
become damaged, a risk of injury or a risk of fire may result, and the warranty will be voided.
