This form acts as a customer receipt and as verification for Digital
Services that the upgrade was completed.

Complete both copies of this form. Then, give a copy
to the customer and a copy to the local CAS office
for filing with customer documents.

Digital will contact you within the next several days
to arrange for package pickup and return. Keep this
copy as your record of installation by Digital.
NOTE
Contact the local CAS office to obtain the RA (return
authorization) number. You should have the Digital
order number available. Be sure to note the name of
the person you speak with.
