Please quote the relevant serial number on all correspondence. There are three
individual serial numbers for the following parts: control unit, top cover and base
cover. UDI and serial numbers can be found on the back of the control unit and
inside the mattress / cushion.
For Service & Support outside the UK & Northern Ireland please contact the local
distribution company from where this equipment was purchased. Failure to do
so may result in the manufacturer’s warranty becoming void.
If a serious incident
occurs in relation to the product, reports should be forwarded to Drive DeVilbiss
Healthcare Ltd and the local competent authority.
