Entering Patient Information
To enter patient information, press the
Patient Information
button on the Home
screen.

1. Patient information.
• Enter the last name.
• Enter the birth date.
• Enter the sex.
• Enter the patient’s ID.
• Enter the first name.
2. Press the up and down arrows to display the second and third data-
entry screen. See the “Entering Patient Information - Second Data-entry
Screen.”
3. Select a referrer from the drop-down menu. To add or delete an item to
or from the list, press add or delete.
4. Select a surgeon from the drop-down menu. To add or delete an item to
or from the list, press add or delete.
