SkyVision Pre-Installation Evaluation

The Pre-Installation Evaluation must be completed prior to the arrival of the SkyVision integration
system installation team. This form can be used to describe a multi-room installation if the
details that are room specific can be noted on the final page. In the event this evaluation is
not completed properly and additional visits are required, the customer will be billed at
$1900.00 per day, per technician.
Pre-Install Inspection
The inspec ti on must be performed by one Skytron SkyVision Technician, the local Skytron Account Representa ti ve,
the on-site customer contact and perti nent contractor representa ti ves.
Pre-install site evaluation
The evalu ati on consists of a review of the project docum entati on during a “walk through” of the en tire project site
and comp leti on of the checklist.

Environmental Considerations:
Do installed Conduit Runs match drawings in both sizing and lo cati ons? Yes or no.
Are any conduit runs missing, changed or di ff erent from the submi tt al drawings? Yes or no.
Are the Nurse Document Centers installed and placed in th eir fi nal lo cati on? Yes or no.
Is there a 12” x 12” box for conduit termina ti on installed at the NDC or Rack loca ti on? Yes or no.
Are the ceilings installed Yes or no.
What is the ceiling material?
Will there be su ffi cient ceiling / mount access to pull cables through conduit? Yes or no.
Is all drywall work fi nished? Yes or no.
Are the fin ish ed fl oors installed? Yes or no.
Has all work involving dust and paint has been completed? Yes or no.
Is HVAC system completed? Yes or no.
Power Considerations:
Will permanent power be live prior to Skytron installers arriving on site? Yes or no.
Are two 120 VAC, 20A, properly circuit protected, dedicated circuits provided to power each
SkyVision system? Yes or no.
Are properly sized NEMA4 enclosures and proper power circuit provided for all monitor power supplies
that are to be placed above the ceiling? Yes, no or NA
